If you are over 18 and decide you do not want your information released to family members, you just need to let your doctors know and they will no longer be allowed to release the information.
Normally a doctor or other health care professional can not give the details of the disease and treatment (or hand over documents) with friends or relatives, unless they have the patient's permission. (except exeption)
