- There are no laboratory studies that are diagnostic for idiopathic NSIP, with some tests being helpful to suggest a possible underlying etiology (e.g., autoimmune serologies, serum precipitins). Positive antinuclear antibodies and rheumatoid factor are present in 43% and 23% of patients with idiopathic NSIP (Travis et al., 2008), although it is not clear whether these patients with abnormal autoimmune serologies should be labeled as having idiopathic NSIP or a subtle form of CTD (i.e., interstitial pneumonia with autoimmune features) (Fischer et al., 2015).
- Given this controversy, patients with an NSIP pattern of apparently unknown etiology should routinely be sent for an extended autoimmune serology panel, including evaluation for myositis-related antibodies.
